Are you interested in participating in a FREE program for teen mothers who are having their first baby? If yes, consider participating in the Chicago South Side Nurse - Family Partnership Program. If you enroll, a registered nurse will visit you in your home, every week or two, throughout your pregnancy and continue to visit until your baby is 2 years old. You and your nurse will decide the exact schedule.
Who can enroll in the program?Any young expecting mother who:
- Is pregnant with her first child (no prior live births)
- 19 years old and younger
- Be less than 28 weeks pregnant
- Meets income requirements (Medicaid or WIC eligible, based on teen’s income alone)
- Lives in the service area (Zip Codes: 60605, 60607, 60609, 60615, 60616, 60617, 60619, 60620, 60621, 60636, 60637, 60649, 60653)

You are encouraged to enroll as early as possible in your pregnancy. An NFP Nurse needs time to visit and obtain consent before the 28th week of pregnancy. Please provide us with information about yourself.
